Our Post Office Box will not change so you may continue to send your Claim
submissions to P. O. Box 52429, Houston, TX 77052-2429. If you must send
documents via overnight delivery, you may obtain our physical address from one of our
Claims Assistance Representatives.
Claim submissions mailed to the SF-DCT:
The SF-DCT will process Claim submissions received through March 13, 2012.
Beginning March 14, 2012 incoming Claim submissions will be date stamped to preserve
the receipt date of your submission and held in house until our move is complete. We
will resume mail processing on April 3, 2012.
Claims Assistance Phone Number:
Our toll free Claims Assistance telephone number will not change. You may continue to
reach a Claims Assistance Representative at 1-866-874-6099.
Access to Claims Assistance:

The Claims Assistance Department will be available to assist you through Monday
March 26, 2012. Beginning Tuesday March 27, 2012 our Claims Assistance Department
will be closed. We will resume normal business hours on Tuesday April 3, 2012
Cure Deadlines:
Cure Deadline dates will be adjusted if your Benefit Claim is affected by the office
closing.

The SF-DCT will respond to e-mail inquiries through Monday, March 26, 2012.
Beginning Tuesday March 27, 2012, our Claims Assistance Department will be closed.
